Diagnosis and Conditions

The system uses the WHO ICD-11 standard for recording diagnoses and conditions. Once lab and radiology results are available, providers review the findings and update the patient's final diagnosis.

Diagnosis tracking

ICD-11 diagnoses are selected from a dropdown directly in the Visit Note. The system tracks both Primary and Secondary diagnoses.

Condition tracking

Conditions are recorded as a longitudinal history with active and inactive dates. Each condition carries a status and the dates it became active and was resolved, which helps track disease progression across visits.

Record conditions from the Condition section of the patient dashboard using the Add button.
