Observation Forms

Clinical information is recorded through customized Observation Forms. These forms are aligned with Nepal's Health Management Information System (HMIS) reporting requirements, so the data captured during routine care also feeds standardized reporting.

The forms capture complaints, symptoms, examination findings, diagnosis, history, and treatment plans, and store them in the patient's EMR.

Forms are shown conditionally based on the patient's age, sex, and visit location. For example, ANC, PNC, and Delivery Notes appear for female patients; Newborn and CB-IMNCI Notes appear based on age; and Dental Notes appear for dental visits.

Use the search bar to find a form. A consultation typically starts with the OPD Visit Note, then adds forms as needed. Once data is recorded, it appears under the Visits section.
